Flu-Clinic Volunteer
Anthem Blue Cross has partnered with Fresno EOC LCC in providing the community with NO COST drive-thru flu vaccinations.The Flu-Clinic Volunteer must sign a waiver and wear a mask at all times. Volunteers are needed on event dates: Oct. 22nd (8:30am-12:30pm), Oct. 29th (12:30pm-4:30pm), Nov. 12th (8:30am-12:30pm) & Nov 24th (12:30pm-4:30pm). Please let the coordinator know of which event you are interested in attending. Duties Include: • Anthem's temp resource arrival with PPE (mask at all times) • Assist/review flu clinic setup (1-2 tents, 1 tables, 1 chairs, 1 garbage can per tent) • Confirm drive-thru setup if applicable • Retrieve waivers and voucher from site partner • Responsible for submitting voucher to CVS • Greeting vaccine administrator (CVS) • Show administrator around site (restroom, etc.) • Confirm with CVS uninsured/underinsured are the only participants eligible for the voucher • Ensure all participants sign waiver/CVS intake form • Hand out giveaways to all participants • Maintain count of number of vaccines by hour • Maintain count of number of vouchers • Gather signed Anthem waivers and report them to community relations representative (Peter.vang2@anthem.com) for pick up • Tear down event resources *Volunteers will mainly be distributing waivers/intake forms and collecting those same forms* Location: Fresno Economic Opportunities Commission (EOC) 1805 E.
